- If you are searching for a Waiver Provider only or you have received any of the below correspondences:
- Select a LOC assessor.
- Select a Case Manager.
- Select a LOC assessor for Reassessment.
Please select yes to the "Are you looking for a Waiver provider only?" question.
- Select a Waiver program that the recipient has been enrolled into from the “Waiver Program Name” drop down.
- Select a Waiver service from the “Looking for a Waiver Service” drop down based on the correspondence you have received.
- After entering search criteria select Search to display results. Please Note: Search results will list providers who are eligible to provide the service but does not guarantee that they offer the selected services.
- Select “Clear” to clear out all fields.
