Substance Use Disorder (SUD) PT 03 Behavioral Health Service Organization (BHSO) Tier Selections and PT 66 Behavioral Health Multi-Specialty Group:  

PT 03:  There are three tiers available for selection:  Tier 1 Mental Health, Tier 2 Outpatient SUD, and Tier 3 Residential SUD.  

PT 66:  No tier selections.  Screen 1.14 is only required for PT 03 Tier 2 if Yes is answered to question ““Are you a licensed Narcotic Treatment Program (NTP)?”

• Enter required NTP address and hours of operation.  Multiple addresses may be entered.  
• Select NTP location or affiliated Medication Station.  Must enter at least one NTP location.
• Enter full street address
• Enter hours of operation – Start Time and Stop Time. Select AM or PM.  Stop time cannot be prior to Start Time.  
• For NTP location, answer “Do you operate a Non-Methadone Clinic at the same location?”  
• If answer Yes, will receive warning message to “Please enroll Non-Methadone clinic as separate entity.”  
• If No, proceed.
