Bank account information that is linked with this Medicaid ID will be displayed on this page. Add, edit or remove the information displayed in the grid. If Payment Type has expired, click the Edit button on record and add Expiration Date.
Payment Type
Select either Check or EFT from the Payment Type drop down
If Check is selected:
Check Payable To field is prepopulated with Provider, Group, or Entity name and cannot be changed on this screen. (To make changes to the Provider, Group, or Entity name use the left navigation to select 1.0 Administrative Information, then choose 1.1 Basic Information. Make the desired changes, then click Save & Next on each subsequent page)
To End Date a previous record, click the “Edit” button in the grid and enter the End Date.
When selecting Check, the Pay-To Provider’s Name 1.1 and Address 1.7 will default; if changes are needed, user must return to screens 1.1 Basic Information to edit Provider/Business Name and/or 1.7 Address to edit the Pay-To Address.
When selecting Check, the Pay-To Address cannot be out of the United States or its territories.
Address is prepopulated with Provider or Entity 1099 address, and cannot be changed on this screen. (To make changes to the Address, use the left navigation to select 1.0 Administrative Information, then choose 1.7 Address. Make the desired changes, then click Save & Next on each subsequent page)
If EFT is selected:
Enter bank Routing Number (RTN). Routing number is the 9-digit ABA (American Bankers Association) code that identifies a particular bank or financial institution. It is found to the left of the account number at the bottom of a check, or on a bank statement
Click Get Bank Details and the following fields will populate from ABA validation at the time of entry:
Bank Name defaults to your branch name from the RTN
Bank Address defaults to your branch address from the RTN
Payee Name defaults to Provider or Entity name
Payee Address defaults to Provider or Entity 1099 Address
In Account Type select either Checking account or Savings account from the drop down menu
Type account number in the Account Number box. (This number may be found at the bottom center of a check, or on a bank statement.)
Verify the account by typing the same number in the Re-Type Account Number box
New EFT records added will show as pending on 6.0 Account Information screen until application is approved by DMS
No change of EFT status can be performed until the current EFT is status is shown as ‘Active’
Three rows will show in the grid; Active, Pending, and EFT Pending
Active- Payment received in requested account
Pending- Payment method is waiting to be approved
EFT Pending- EFT information is waiting to be verified
No changes can be made to EFT information until the Active, Effective Date is reached
EFT records will require voided check or bank statement to be uploaded in the “8.0 Document Upload” screen. If the provider doesn’t have EFT ability, Check will need to be selected.
End Date:
In KY MPPA, some records cannot be deleted. To add a new record, the previous record must have an End Date added in order to keep the history on file. To add an End Date to a record, click the "Edit" button and add an End Date. If a new record is added to the grid, the active dates of the new record cannot overlap existing record dates.
Required Fields
Please note that all fields marked with an asterisk (*) are required fields that must be populated in order to move forward. If they are left blank you will receive an error message.
Special Characters:
The following Special Characters cannot be used in the situations below. Field properties will not allow the following characters in the box:
For First Name, Business Name, Other name (if any): | + \ '
For Last Name field: | + \ ', and numbers are not allowed
For Comment Boxes: < > ^ ~ ` + | \ / ¬ £
For Address fields: | + ! @ # $ ^ * ( ) ? / \ " < > & : ; _
If you copy and paste an excluded special character, an error message will display after clicking Save & Next
Remove excluded characters from pasted information or uploaded text in the document
Save & Next:
When finished, click Save & Next to save the information and proceed to the next page
Click Back to go to the previous screen without saving
Click Exit to return to the dashboard without saving
Note that if the information is not saved, you will lose the data on the current page
Additional Notes:
Addition of EFT information is optional. (If EFT information is not provided, is not complete, or the status is not active the payment type will default to check when actual payments are made)
No Out of Country EFT accounts are allowed
No Out of Country Pay To addresses are allowed
