Use the Provider Review screen to verify and correct all keyed screens/fields after data entry is completed and before submission.
View the information in each section of the Application by expanding each topic heading. Expand by clicking the name of the topic or the left arrow in the title.
Only one section will expand at a time for review
To make corrections, use the left navigation menu to jump to the section in the Application that needs corrections. To expand the sections in the left Navigation Menu, click the topic name or the left arrow.
Any time an applicant returns to a screen previously completed and makes a change, they are required to select Save & Next on every screen to get back to where they previously were’ before selecting submit. Save & Next validates each screen subsequent to the change to identify dependencies and provides an error message if an update is required.
In the Provider Application Level Comment field at the bottom of the Provider Review screen, the Provider may enter comments, up to 4000 characters, excluding identified special characters that are not allowed in comments. (see the Additional Notes section below).
Click Save & Next to continue to next step
After final submission, no further changes can be made to the Application unless the Application is returned for corrections or is approved
If DMS returns an Application for corrections, any comments DMS enters into the DMS to Provider field will be visible in a new grid displayed on the screen.
Providers can send comments back to DMS.
Comments will be sorted by most recent first, then by Comment Number sorted most recent first
External user names will appear in the Comment Updated By column
Click Save & Next to continue
Required Fields:
Please note that all fields marked with an asterisk (*) are required fields that must be populated in order to move forward. If they are left blank you will receive an error message
Special Characters:
The following Special Characters cannot be used in the situations below. Field properties will not allow the following characters in the box:
For First Name, Business Name, Other name (if any): | + \ '
For Last Name field: | + \ ', and numbers are not allowed
For Comment Boxes: < > ^ ~ ` + | \ / ¬ £
For Address fields: | + ! @ # $ ^ * ( ) ? / \ " < > & : ; _
If you copy and paste an excluded special character, an error message will display after clicking Save & Next
Remove excluded characters from pasted information or uploaded text in the document
Save & Next:
When finished, click Save & Next to save the information and proceed to the next page
Click Back to go to the previous screen without saving
Click Exit to return to the dashboard without saving
Note that if the information is not saved, you will lose the data on the current page
